Please let us know if you change your name, address or telephone number.
Please do everything you can to keep appointments. Tell us as soon as possible
if you cannot attend so we can offer it to another patient.
For x-ray and ultrasound results please be patient as these take time
to reach us. Please ring about 14 days after the test for the results. If the
test was ordered by the hospital, enquiries should be directed to the hospital,
not the practice.
If you are referred to a specialist, please understand we have no control over
the speed in which you will be seen.
Suggestions & Complaints
Suggestions are always received with pleasure and certain consideration.
If you have any complaints then one of our supervisors will be available on reception
(wherever possible) and will be more than happy to help. If you feel the matter
has not been resolved, we would ask you to put your complaint in writing to Dr
Elaine Tamkin or Mrs Lyndsay Rodway, who will process the matter through our
in-house complaints procedure.
Zero Tolerance
Our doctors and staff have the right to care for others without fear
of being attacked or abused. Patients who are violent or abusive to any member
of staff will be removed from our list.
Freedom Of Information
The Freedom of Information Act 2000 obliges the practice to produce
a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of
information the practice intends to routinely make available. This scheme is
available from reception.
Data Protection
Patients may have access to their health records on payment of the current
statutory fee. Other healthcare professionals who would require access to these
records as part of providing treatment, will also be able to access them.
Any other requests for access to a patient’s health records must be accompanied
by a signed letter of consent from the patient otherwise we cannot release records.
Patient records in this practice are sometimes examined for the purpose of medical
audit. If you have any objection to this please notify the practice manager.
Patient information is stored on the computer. This is accessed only by Springfield
House staff and all records are subject to the Data Protection Act.
