How Do I.... Register With The Practice?

In order to register at the practice, applicants are required to complete a registration form available at reception and an appointment will be offered for a new patient health check. Each new patient over the age of 10 years will be required to undertake a health check with our practice nurses prior to registering with the practice. The registration forms need to be completed and brought to the appointment with a urine specimen.
Patients who do not attend the health check appointment will not be offered a further appointment and their application will not be accepted.
